If you do not have a pre-approved purchasing account with us, please download and complete the account packet below and fax it to 512.519.2408 or email it to BulkInquiries@blueskyscrubs.com.

Three easy steps to complete your order via purchase order:

1. Log in to your pre-approved purchasing account.
2. Upload your purchase order below.
3. Continue to your shopping bag and checkout. That's it! We will take care of the rest! You will receive a confirmation email, shipping tracking number, and an invoice for the items listed on your purchase order.

Alternatively, you can fax your purchase order to 512.519.2408 and we will enter in your order after receipt of your fax.
